Yearly MHA DOT Decal
Required Information
- Certificate of Liability Insurance with MHA DOT listed as the certificate holder using the address:

New Town ND 58763 - Certificate of Liability Insurance must have both General Commercial Liability & Auto Liability in a minimum of one million coverage. Hazardous Material hauling requires a five million dollar coverage.
- Current vehicle registrations that are in the company name or capital lessor’s name with attached lease agreement (Will accept the purchase documents or the temporary registration for a new vehicle)
- Current TERO Business License – If you are trucking only, you will get a license from the MHA DOT office (not tero) and select the area in which you “need a tero license” on this application.

Personal Vehicle Decal
Required Information
- Employer or Consultant’s TERO Business License
- Current vehicle registration
- For Consultants a Certificate of Liability Insurance with MHA DOT listed as the certificate holder
- For Employees personal Auto Insurance card and the Certificate showing Commercial General Liability coverage
- Driver’s License
- (If Employee} – Employment Verification: W2 or most recent pay stub

Temporary Rental Permit
Required Information
- This tag is only for short term rentals for company men or consultants
- You will log into your online account and fill out a Transportation Application same as a employee
- TERO Business License for Employer
- Rental Agreement from a bonafide leasing agency
- Clients Driver’s License
- COI of Insurance covering the vehicle.
HANG TAG WILL BE ISSUED WITH EXPIRATION DATE.
Used only when the vehicle is a rental from a verified rental company. If the permits need to be extended, please call the MHA DOT office.
Fee
$50 for 30 day rental
$100 for anything longer then 30 days.
Native Owned Company
Required Information
- Certificate of Liability Insurance with MHA DOT listed as the certificate holder using the address:

- Current vehicle registrations that are in the company name or capital lessor’s name with attached lease agreement.
- Current TERO Business License.
Fee
Depends on Tier Type given from the TERO Office & should be listed on the Native Contracting License. Minimum $100 per 5 vehicles – Maximum $500 per 5 vehicles.
Trucking Only Applicants
Trucking only companies will need to apply online for their TERO Business License and their application will be approved at the monthly TERO Commission meetings, every second Tuesday of the month.
New Trucking Licenses are $1000.00 per applicant. *credit card transitions are subject to a 3% minimum credit card fee.
Trailers do not need decals
Employee Personal Vehicle
Employee’s personal vehicle needs to be a standard vehicle. Semi’s will not be able to get a hanging tag. Verified Employees are only allowed up to one vehicle per employee. If you need more vehicles to register you will be charged for up to 5 vehicles. Each Personal Vehicle that will be on an oilfield related location will need to be registered with the MHA DOT.
Leased Vehicles
There are two types of classifications:
Owner-Operator Lessors
Trucking companies that are working for another company. These companies will need their own TERO ID and stickers. NO EXCEPTIONS!
Bonafide Lessors
A corporate leasing agency that only rents our their vehicles as a service. (example: enterprise trust, don-len trust, etc.). These vehicles can be added to the account with a copy of the lease agreement

Any company related to oil traffic doing business within the Fort Berthold Indian Reservation Boundaries.
Exemptions:
- The government of the Tribes or any subdivision or enterprise of the government of the Tribes, to the extent of the Tribes’ ownership
- Farming and livestock activities occurring on Reservation lands
- Charity, church, or school groups
- Persons engaged in primarily in the sale of food stuffs
- First responders and/or emergency response crews
Each native-owned company must go through TERO Commission in order to get their license. We do not register any native-owned company without the commission’s approval. Each native-owned company pays a yearly fee depending on their status with TERO. See the Registration Specialist for more info.
Trucking Brokerage is defined as a company that does not own vehicle but dispatches owner-operators to locations. Yes they need to be registered with the TERO office, not MHA DOT.

You can request your username by emailing or contacting the registration specialist or anyone at the MHA DOT office. We do not keep password information but we will be able to help you get a new password.
Call the office and any of our employees will be more than happy to get a credit card over the phone or change your online credit card in the system
Select the vehicle type as “OTHER” -this is an system glitch that we are currently trying to figure out.
Delete the files and select the FILE TYPE as VEHICLE REGISTRATION, CERTIFICATE OF INSURANCE OR TERO ID-NOT OTHER
The application asks if you would like the tags mailed or picked up. If the tags are mailed they are mailed via USPS & may take up to 7 business days to arrive. If you pick the tags up please allow one business day to process.
The tags can take up to 10 business days. PLEASE PLAN AHEAD!
